Checklist for documents

CHECK–LIST FOR FRESH AND RE-ISSUE OF OCI CARD

The following documents are attached with the application (Please tick mark the document attached with the application).
Printed copy of the duly filled application form at [https://ociservices.gov.in/]. (The application should be signed by the Applicant in the given box/thumb impression).
Two (2) recent photographs [50x50 mm].
Self-attested copy of the current passport.
Self-attested copies of the last cancelled Indian passport.
Self-attested copy of Nationality Certificate.
Self-attested copy of the Certificate for Renunciation of Indian Citizenship (adults)/ Certificate for surrender of Indian Passport (minors).
Self-attested copy of the address Proof.
Self-attested copies of the marriage certificate, spouse passport and residence permit (In case if you are married).
Current passports and resident permits/ID cards of parents (In case of minors and applicants who are born outside India).
International Birth Certificate (Duly apostilled/attested by the authority concerned in case the applicant born outside India and never held Indian passport).
International Marriage certificate (In case of foreign spouse, duly attested/apostilled by authority concerned).
Marriage certificate of the Parents (in case of minors and applicants born outside India. If the parents are divorced, court order of dissolution of marriage, which specifically mentions that the legal custody of the child is with the parent who is applying for the OCI card).
Name change certificate in case of change of name.
Copy of PIO card (In case of PIO to OCI).
Police report containing the OCI/PIO Card number (in case lost OCI/PIO).
Undertaking A (For minors and applicants born outside India).
Undertaking B & C (In case of Foreign Spouse).
One pre-paid self-addressed envelope (Size A5) with postage stamps worth € 4.25 pasted.
